QIP performance score cards must be posted by Dec. 22

The performance score cards for payment year 2012 that were published on the Dialysis Reports website on Dec. 15, 2011 must be printed and posted no more than five business days after the date of publication.  
 
These score cards contain comparative data that shows how well a dialysis facility’s total performance score compares to the national performance rate, identifies the performance score achieved on each measure, and also shows how well the facility individual performance scores compare to the national performance rate for each measure.  This information will also be posted on Dialysis Facility Compare in January, 2012.

Posting instructions

To obtain your score card, log on to DialysisReports.org and sign in using the user name and password you were previously given for your Dialysis Facility Reports. Print the score card on plain, blank, white or light colored paper, no smaller than 8"x11". Post the score card prominently in the patient waiting room.
