Dialysis provider Fresenius Medical Care announced that it is partnering with ApolloMed ACO to provide integrated health care management for its patients with end-stage renal disease.

As part of the agreement, ApolloMed ACO and Fresenius Medical Care will share in operations and financial cost savings from the partnership.  The multi-year agreement is built on the five-year demonstration project that Fresenius Health Partners, a subsidiary of Fresenius Medical Care, conducted with Medicare beneficiaries on dialysis.

Fresenius also announced in April that it would provide integrated health care management for Heritage California ACO (HCACO) patients who have End Stage Renal Disease. HCACO is a pioneer accountable care organization. The agreement supports a population health program that allows Fresenius Medical Care to work with HCACO in managing the overall health and coordination of care of these HCACO beneficiaries, with the goal of improving their health outcomes and reducing their total cost of care, according to a Fresenius press release.

This population health program is also modeled on a five-year demonstration project that Fresenius Health Partners conducted with Medicare beneficiaries on dialysis.
